Practice Information:  Medical Certificates/Reports


 


Medical Certificates

For the first seven days that you are absent from work a self certification form should be completed. These forms are often available from your employer. If they are not available or you are self employed then you can obtain one from reception at either of our sites or from a Post Office.

If you need a certificate for any additional time you will need to make an appointment to see a doctor. Further certificates can then be arranged with one of our receptionists. Where possible please contact the surgery two days before your sick note runs out (no later than the day before please).

You will be asked:-

Your full name and address
The date you would like your sick note to start from
Your medical condition - this information is on your current sicknote
The duration that the sicknote will cover
The GP who is dealing with your current medical condition

Please allow 48 hours before you collect your certificate. We would advise that you telephone reception before you come in to collect your sicknote.

Medical Reports

Our doctors will sign and complete various forms on your behalf. Please be aware that these often carry a charge. The receptionist can advise you if you wish to know the cost of your particular form.

Please hand your form into reception at Brixham or post it to our Brixham address. If you hand a form in to reception at Galmpton, the receptionist will send your form to Brixham to be actioned.

Please be aware that forms cannot be treated as priority over consulting ill patients but the doctor will endeavour to complete them within 5 working days of receipt at the Brixham surgery.
